New Patients:
We welcome you to the Beach View Dental family. You will need to complete the forms below at least a day in advance. Once completed, you will be able to submit via the web. If you have any problems, please feel free to arrive at your appointment at least thirty minutes early, and you will be able to complete the forms in the office.
Patients:
With the log-in below, you have the ability to view your account, see your treatment plan, dates/times of upcoming appointments and the ability to pay on account. The first time you visit the log-in, you will need to create your sign-in information and the next business day we will connect your sign-in information with your account. Please allow at least 48 hours to have your sign-in activated. After that you will have the ability to visit your information anytime.
